Ordering Supplies
Our group furnishes all supplies necessary for the collection and transportation of specimens to our laboratory at no additional cost.
To order supplies, you may use the form below or you may download a PDF version. Please fill out and fax the PDF version to: 615-222-3702.
If you have ordered with this form before, you can use the customer ID provided at that time to bypass the re-entry of your name, address etc. If you don't recall your customer ID, or have not previously used the form, you can always simply enter your contact information — the customer ID is a convenience, and not required.
